AGREEMENT & TERMS OF SERVICES
3E.LIFE, LLC AGREEMENT
1. I understand and agree that I am fully responsible for my physical, mental and emotional well-being, including my choices and decisions. I am aware that I can choose to discontinue 3E.LIFE, LLC services at any time.
2. I understand that 3E.LIFE, LLC does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association. I understand that 3E.LIFE, LLC is not a substitute for psychotherapy, psychoanalysis, mental health care or substance abuse treatment and I will not use it in place of any form of diagnosis, treatment or therapy.
3. I understand that information will be held as confidential and 3E.LIFE, LLC will not disclose any information pertaining to the sessions unless stated otherwise and except as required by law.
4. I understand that my information is not protected by legal confidentiality (like doctor-patient or attorney-client). As such, 3E.LIFE, LLC could be required to divulge otherwise confidential information to authorities.
5. I understand that certain topics may be anonymously and hypothetically shared with other professionals for training or consultation purposes.
6. I understand that 3E.LIFE, LLC is not to be used as a substitute for professional advice by legal, medical, financial, business, or other qualified professionals. I will seek independent professional guidance for legal, medical, financial, business, or other matters. I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my sole responsibility.
7. I understand and agree to be present with no external distractions, communicate honestly, be open to feedback and suggestions, which includes sharing suggestions on how I can get more value from the sessions.

3E.LIFE, LLC TERMS OF SERVICES
Description of Service 3E.LIFE, LLC is a partnership (defined as an alliance, not a legal business partnership) between 3E.LIFE, LLC and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation/development of personal, professional, or business strategies and to develop and carry out a plan for achieving those outcomes.
1) 3E.LIFE, LLC-Client Relationship:
A. 3E.LIFE, LLC agrees to maintain the ethics and standards of behavior established by the 3E.LIFE, LLC Agreement. It is recommended that the Client review the 3E.LIFE, LLC Agreement located on the website 3E.LIFE/coaching-agreement
B. Client is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions, and results arising out of or resulting from the 3E.LIFE, LLC relationship and his/her calls and interactions with 3E.LIFE, LLC. As such, the Client agrees that 3E.LIFE, LLC is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. The client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
C. Client further acknowledges that he/she may terminate or discontinue the 3E.LIFE, LLC relationship at any time.
D. Client acknowledges that the 3E.LIFE, LLC is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education, and recreation. The Client agrees that deciding how to handle these issues, incorporating principles into those areas, and implementing choices is exclusively the Client’s responsibility.
E. Client acknowledges that 3E.LIFE, LLC does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If the Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the 3E.LIFE, LLC relationship agreed upon by the Client and the Coach.
F. The Client understands that in order to enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback and assistance, and participate fully in the program.
2) Procedure: The time of the sessions/meetings and/or location will be determined by 3E.LIFE, LLC and the Client based on a mutually agreed upon time. The Client will initiate all scheduled calls and will contact the Coach for all scheduled meetings.
3) Cancellation Policy: The client agrees that it is the Client's responsibility to notify 3E.LIFE, LLC 24 hours in advance of the scheduled calls/meetings. 3E.LIFE, LLC reserves the right to bill the Client for a missed meeting. 3E.LIFE, LLC will attempt in good faith to reschedule the missed meeting.
4) Limited Liability: Except as expressly provided in this Agreement, 3E.LIFE, LLC makes no guarantees, representations, or warranties of any kind or nature, express or implied with respect to the 3E.LIFE, LLC services negotiated, agreed upon, and rendered. In no event shall 3E.LIFE, LLC be liable to the Client for any indirect, consequential, or special damages. Notwithstanding any damages that the Client may incur, 3E.LIFE, LLC entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to 3E.LIFE, LLC under this Agreement for all coaching services rendered through and including the termination date.
5) No Guarantee: The client acknowledges and agrees that 3E.LIFE, LLC cannot guarantee the results or effectiveness of any of the services rendered or to be rendered. Rather, services shall be executed in a professional manner and in accordance with good industry practice. Best efforts will be used but no results are promised.
6) Entire Agreement: This document reflects the entire agreement between 3E.LIFE, LLC and the Client and reflects a complete understanding of the parties with respect to the subject matter. This Agreement supersedes all prior written and oral representations. The Agreement may not be amended, altered, or supplemented except in writing signed by both 3E.LIFE, LLC and the Client.
7) Dispute Resolution: If a dispute arises out of this Agreement that cannot be resolved by mutual consent, the Client and 3E.LIFE, LLC agree to attempt to mediate in good faith for up to 30 days after the notice is given. If the dispute is not so resolved, then the dispute may proceed with legal action.
8) Severability: If any provision of this Agreement shall be held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. If the Court finds that any provision of this Agreement is invalid or unenforceable, but that by limiting such provision it would become valid and enforceable, then such provision shall be deemed to be written, construed, and enforced as so limited.
9) Waiver: The failure of either party to enforce any provision of this Agreement shall not be construed as a waiver or limitation of that party's right to subsequently enforce and compel strict compliance with every provision of this Agreement.
10) Binding Effect: This Agreement shall be binding upon the parties hereto and their respective successors and permissible assigns.
